Separate records prevent separate failures

A commitment is the outcome still owed. An assignment records responsibility. A claim grants temporary exclusive execution. An attempt records one run. Evidence explains why a completion decision is justified.

Assignment is not a claim.
A successful attempt is not a completed commitment.
An output is not automatically evidence.
An approval is not an executed external effect.
A provider timeout is indeterminate, not safely retryable failure.
